Register by May 30 for June 23-24, 2009 Michigan Teen Conference at CMU
Do you know a youth in Foster Care or Independent Living who needs to learn about bank accounts, meal planning, educational opportunities, healthy relationships, or housing? If you do, then you need to register them for this 2009 Michigan Teen Conference. Youth must be ages 14 to 21 and in Foster Care or Independent Living. If they are under the age of 18 they must have a chaperone. Caretakers of the youth and professionals who work with teens are also invited. Attendees will sharpen their understanding of what is available and required for foster youth to make the challenging transition to adult life.
This conference is also an excellent place for foster parents to earn training credit hours.

Click here if you would like to download a Word file to type into the Registration Form and Two Consent Forms per youth.
IMPORTANT:
Make your request for YIT funds ASAP; a worksheet to help with making this calculation is provided in the brochure on page 2.
Full payment to fiduciary LSSM must be made by May 30, 2009.
UPDATE: Despite the Michigan Teen Conference brochure stating that the MCI Superintendent must sign the Release/Consents that are provided with the registration forms, it has been decided that the primary worker for a youth can sign these, instead of the MCI Superintendent.
There have been discussions with the MCI Superintendent, and he has stated that the primary worker signature, whether this be DHS or Private Agency, is sufficient for the situation. Therefore, you do not need to fax these consent forms to the MCI office. For any forms that are faxed to the MCI Superintendent, an email will be sent to notify the worker regarding this new information.
